(PRACTICE TRANSOFORMATION) The Director Population Health
Management will lead and manage intermediate to complex population
health projects and implementation efforts and manage a
multi-disciplinary team. In this role, the Director will work
closely with the Program Director(s) and Executive Leads to oversee
program management. This position will report directly to the
Associate Vice President. During the COVID pandemic, this role will
be mainly remote with some travel to the office as needed. This
Director role is responsible for assisting with assigned
Medicalincs Population Health Management programs & Business
Development. Specific duties include but are not limited to: Create
& maintain project documents, including the project plan, project
schedule, project charter, and project scope, maintaining meeting
summaries, risks/issue log, and RACI Matrix. Execute certain
aspects of the Project tasks & Follow-up with tasks owners as
needed, and facilitate meetings as needed. Coordinate and manage
external and internal communications and resources to maintain
quality service delivery. Managing relationships with key clients
and stakeholders (in a multidisciplinary setting). Review annual
compliance document and provide summary on changes. Assist with the
development and delivery of program compliance & operations
training. Prepare program agreement documents as well as assist
with practice recruitment efforts. This Director role will assist
with Business Development activities when identified.
RESPONSIBILITIES & COMPETENCIES
- Responsible for leading & managing key projects/programs.
- Create & maintain project documents, including the project
plan, project schedule, project charter, and project scope,
maintaining meeting summaries, risks/issue log, and RACI
Matrix
- Execute certain aspects of the Project tasks & Follow-up
with tasks owners as needed, and facilitate meetings as needed
- Coordinate and manage external and internal communications and
resources to maintain quality service delivery
- Managing relationships with key clients and stakeholders (in a

- Review annual compliance document and provide summary on

- Assist with the development and delivery of program
compliance & operations training
- Prepare program agreement documents
- Serve as a Consultant to Primary Care Practices on process &
care delivery improvement
- Lead Project Quality Improvement efforts & Change
Management
- Support with Data Analytics process by working closely with
Data Analysts to prepare data files & analyze data
- Prepare program summary and executive reports for leadership
review
- Other duties as assigned QUALIFICATIONS
- Bachelor's degree in business administration, public health, or
management; Masters preferred
- Experience in the healthcare industry required
- Project Management Professional (PMP) certification required
(or acquired within 1 year of employment)
- 2 - 3 years of project management or related experience
- Experience with practice transformation and community health
best practices
- Lean Sigma certification preferred (or can be obtained within 6
months of employment)
- Excellent organizational, prioritization, and interpersonal
skills
- Written and verbal communication skills
- Ability to successfully manage a project team virtually
- Capacity to manage high-stress situations
- Ability to multi-task and manage various project elements
simultaneously

- Strong familiarity with project management software tools,
methodologies, and best practices
- Intermediate to Advanced technical skills in Microsoft office
suite: Teams, Word, Excel, and PowerPoint; Databases, and knowledge
of EMRs, Project management applications (e.g. Microsoft Project,
Smart sheet, etc.)
- Must be able to Telework from Home Based Office when needed.
Must have a high-speed Internet connection and using tools to
communicate (verbally & written) remotely In addition to the above
qualifications the successful incumbent is expected to consistently
demonstrate:

- Flexibility and the ability to adapt to the changing healthcare
environment and legislation. REQUIREMENTS Work is generally
performed within an office environment, with standard office
equipment available. Work may be performed at the client site
and/or Medicalincs office. Local travel may be required for the
purpose of training, meeting with clients, stakeholders, or
off-site personnel/management. Work is mostly sedentary in nature,
but may require occasional standing and walking. Lighting and
temperature are adequate, and there are no hazardous or unpleasant
conditions caused by noise, dust, etc. Must have good vision and be
able to read, write, and communicate fluently in English. Optimal
time management is required to meet project deadlines within
