Job Summary
Under minimal supervision, the Breast Health Nurse Care Coordinator
functions as a liaison between the provider and patient to achieve
optimal clinical and quality outcomes. The Breast Health Nurse Care
Coordinator is a patient advocate, educator, and counselor for
patients seeking services through the MedStar Health System.
Advocates on the patient's behalf to ensure their needs are met and
to promote optimal patient experience. Demonstrates ability to
establish and maintain effective relationships with providers,
support services, patients, and their families through active
listening, empathy, courtesy, and professionalism.
Minimum Qualifications
Education/Training
Bachelor's degree in nursing required
Experience
3 years of related nursing experience working in a hospital or
medical office setting.
License/Certification/Registration
Valid RN license in the District of Columbia, Maryland or Virginia,
or any combination as required based on work location(s). CPR
certification required. Certification as Breast Health Nurse,
preferred.
Knowledge, Skills & Abilities
Excellent customer service/patient interaction skills; ability to
use computers and navigate the electronic medical record. Ability
to establish and maintain effective working relationships.
Excellent interpersonal skills and telephone/written communication
skills. Committed to providing excellent service to MedStar
customers and ability to display strong listening skills and
empathy to MedStar patients and co-workers. Must be able to work in
a fast-paced, often high-pressure environment, with proven ability
to maintain composure in stressful situations and manage and
diffuse angry or upset patients. Must be self-directed and able to
multitask. Ability to operate standard office equipment.
Primary Duties and Responsibilities
Contributes to the achievement of established department goals and
objectives and adheres to department policies, procedures, and
quality and safety standards. Addresses roadblocks to achieving
goals and objectives.
Ensures compliance with governmental regulations and accreditation
standards.
Demonstrates behavior consistent with MedStar Health mission,
vision, goals, SPIRIT values, objectives and patient care
philosophy.
Reviews electronic health records and identifies patients who would
benefit from the breast health screening program. Collaborates with
providers in establishing appropriate follow up plans for these
patients.
Supports the patient with further care coordination with MedStar
Health. Schedules and accompanies patient to appointments and
procedures, if indicated.
Collaborates with attending physicians regarding individual patient
care by assisting in the coordination of the patient care by
facilitating appointments for procedures and physician consults.
Assists patients to overcome barriers to care with appropriate
referrals such as but not limited to transportation.
Promotes positive relationships with all healthcare disciplines to
ensure timely appointments, results reporting and communication
regarding patient care and follow-up.
Responsible for outreach efforts to establish and maintain positive
working relationships with key customers (physicians, office staff,
diagnostic staff, radiology staff, etc). Monitors patient progress;
consults with providers as needed and refers to appropriate
services.
Maintains collaborative professional working relationship with
clinical and administrative care team members necessary to develop
and maintain a successful cross-continuum care necessary for this
patient population.
Participates in multidisciplinary quality and service improvement
teams as appropriate. Participates in meetings, serves on
committees, and represents the department and facility in community
outreach efforts as appropriate.
Supports own personal and professional development needs to
practice at a high-level of independence.
Participates in community outreach efforts.
Develops and distributes weekly reports of activity. Monitors
outcomes and productivity measures in area of responsibility.
Performs other duties as assigned.
