Job Description:
SummaryJob Summary:Provides direct and indirect patient care
optimizing patient safety. Works proactively and in collaboration
with other team members. Performs comprehensive history and
thorough physical examination. Assess patients cognitive ability
and capacity to understand the anticipated surgery. Screen patients
for depression and risk factors for delirium. Document functional
status and history of falls. Determine baseline frailty score.
Assess nutritional status. Obtain accurate and detailed medication
history. Determine patient's family and social support systems. Act
as a liaison for patients/families and participate in discharge
planning, and serves as a liaison for patient's primary care team
Performs various diagnostic procedures such as bacteriological
stains, smears, cultures, drawing blood, and urinalysis. Orders
laboratory tests, x-ray studies or other diagnostic tests as
appropriate. Establishes therapeutic or corrective measures as
related to specific illnesses or anticipatory guidance. Refers
patient to appropriate licensed physicians or other health care
providers. Coordinates care with specialty clinics, physicians,
social agencies, schools, governmental agencies, etc. on behalf of
the patient. Maintains and reviews patient records. May track and
report patient data to monitor clinical outcomes. Performs
in-patient Geriatric consults and daily multidisciplinary care in
collaboration with Surgeons, Geriatricians, Hospitalists, and
primary care Screen patients for geriatric vulnerabilities.
Recommend various diagnostic procedures such as bacteriological
stains, smears, cultures, drawing blood, and urinalysis. Recommends
laboratory tests, x-ray studies or other diagnostic tests as
appropriate. Establishes therapeutic or corrective measures as
related to specific illnesses or anticipatory guidance. , and
refers patient to appropriate licensed physicians or other health
care providers. Coordinates care with specialty clinics,
physicians, social agencies, schools, governmental agencies, etc.
on behalf of the patient, maintains and reviews patient records,
and tracks and reports patient data to monitor clinical
outcomes.Requirements:Current Maryland CRNP License
